Having health insurance is a requirement for all foreigners, including both EU citizens and non-EU nationals, who want to reside in Spain for more than 90 days. This applies to various types of residency permits, such as non-lucrative visas, temporary residence permits for internships or students, and long-term residence permits. 

The health insurance must be obtained from an health insurance company authorized in Spain. 

The health insurance for Spanish residency must not have co-payments (sin copagos), which means there are no limitations, and the insured individual will not have to pay any additional costs each time they use medical services or waiting periods (i.e. it must offer immediate access to procedures). 

Additionally, the immigration authorities require repatriation insurance, which ensures that in the event of the insured person's death, the insurance company is responsible for handling all the paperwork and covering the costs of transporting the body to their country of origin. 

In co-payment insurance, the insured has to pay an additional amount, in addition to the insurance premium, to use the services. However, without co-payment, the insured does not have to pay any extra fee, only the cost of the premium.
